Sub: Acknowledge & Adherence of ETPL Code of Conduct policy.

I would like to submit my candidature for the profile of a CSE /CRA/ Sr. CRA with Epicenter Technologies. By signing this document I acknowledge to the below mentioned points:

• I will ensure that I am punctual to work and any delays in reporting to work will be against the compliance policies at Epicenter
• I will be on a probation period for the first 6 months of my tenure with the organization
• Apart from my rostered weekly leaves; I will not be eligible for any PL’s for the first 6 Months of my tenure with the organization. Based on eligibility and operational/HR approval, I may be given a CL - in case of any emergency during this timeframe
• Domestic Process :
  • The process that I am applying for is a 6 Day working process with one rotational work off
  • I am aware of the shift timings which may be rotational
  • I am aware that no transport facility will be provided by the company
  • I am aware I will go through a full day training, in case I prefer working for part time
• International Process :
  • The process that I am applying for is a 5 Day working process, However I can be expected to work on the 6th day or extend shifts on any given day, due to volume increases. Which is Compulsorily as per business requirements
  • I am aware of the shift timings and agree to be trained and work across multiple queues including a 24x7 shift, as the business requires

I clearly understand that the Home pickup and drop service is applicable for all shifts commencing or concluding only between 11PM to 5AM IST. Additionally, I clearly understand the transport guidelines and the transport boundary limits. (Pick up & Drop - Boundary Limits:- * “Virar to Dadar”, * “Thane to Dadar”, * “Mankhurd to Wadala Road”)

• I am completely aware about the process that I am have been selected for
• I will not be given any preference in regards to shift timings and weekly offs
• I will at all times ensure that I maintain the decorum of the organization by maintaining a disciplined approach and professional conduct
• Pan card & Aadhaar card are mandatory documents, failing to produce it will result in a hold on my Salary
• I understand and acknowledge that the company would be spending lot of monetary resources on the training and induction of an employee, for which the cost is identifiable. In addition, the following has already been explained to me:
  1. Full time employees will be eligible for a training allowance of Rs 1000(weekly) during training phase, in case they do not certify or graduate from training
  2. Part time employees will be eligible for a training allowance of Rs 500 (weekly) during training phase, in case they do not certify or graduate from training
  3. If you are certified or have graduated, you will be eligible for your normal salary, as discussed during the interview
